Utilize excellent communication skills to build rapport with members, providers, and community resources.
Conduct assessments and develop individualized care plans for members based on their needs.
Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care for members.
Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Educate members on their healthcare options and resources available to them.
Maintain accurate and timely documentation of all case management activities.
Adhere to all company policies and procedures, as well as regulatory standards.
Stay up-to-date on industry trends and best practices in case management.
Participate in team meetings and trainings to enhance skills and knowledge.
Act as a liaison between members and healthcare providers to ensure timely and appropriate care.
Advocate for members' rights and needs to ensure they receive quality care.
Develop and maintain relationships with community resources to provide additional support for members.
Collaborate with other Case Management Coordinators and healthcare professionals to share best practices and improve processes.
