We are looking for a committed Telemedicine Physician to join our team in a fully remote role. In this position, you will provide comprehensive care and support to patients managing hypertension, diabetes, and other chronic conditions, including conducting initial consultations, assisting with ongoing HTN/DIA management/escalations, and facilitating enrollment in our Remote Patient Monitoring (RPM) and Chronic Management programs. The ideal candidate will hold multiple state licenses, with a preference for 20+ states (e.g. Florida, Texas, NY, NJ, California is a must). If not already licensed in all states, you must be willing to obtain the necessary licenses. This role requires a commitment of five days per week (20 to 40 hours), with an expectation of managing approximately 15 patient encounters per day.
Key Responsibilities:
• Conduct telemedicine consultations with chronic care condition patients, offering personalized care and support.
• Perform initial patient encounters and enroll eligible patients in the Chronic management programs.
• Order devices for patients who meet the criteria and request this service.
• Provide guidance and adjustments to treatment plans for patients whose levels are not adequately controlled.
• Collaborate with patients to enhance their chronic conditions management without replacing the relationship they have with their Primary Care Physician and/or specialists.
• Work closely with our nursing team and other team members.
Qualifications:
• Must be licensed to practice medicine in the U.S. and eligible to provide telemedicine services in multiple states. Must have Nevada state license since 2019 or prior, and be willing to obtain all state licenses.
• Must reside within the United States or a U.S. Territory.
• Experience in chronic care management and Remote Patient Monitoring,* is preferred*.
• Strong communication skills and ability to provide compassionate, patient-centered care remotely.
• Comfortable working remotely with a flexible schedule, including some evening and weekend hours.
Physical Demands:
• Prolonged Sitting: The role involves sitting for extended periods while conducting virtual consultations, reviewing patient records, and completing documentation.
• Manual Dexterity: Frequent use of a computer, keyboard, mouse, and other office equipment is required for performing job duties, including data entry and managing patient information.
• Visual Acuity: The job requires good vision for reading patient records, reviewing data on screens, and conducting video consultations with patients.
• Communication: The ability to speak clearly and listen carefully during patient interactions and team meetings is essential, ensuring effective communication.
Work Environment:
• This role is performed in a remote environment, typically in a home office setting. Candidates should have a quiet, private workspace to conduct patient consultations without distractions.
