Job Summary:

Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.

Qualifications

Qualifications:
• Licensed Vocational Nurse, per the state requirements.
• Good communication skills, assessment skills, and strong clinical documentation skills.
• Ability to read, write, and speak the English language.
• Reliable transportation. Valid and current auto liability insurance.

Essential functions:
• Under the direction of the RN, assist in identifying the patient's physical, psychosocial, and environmental needs.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Provide care utilizing infection control measures that protect both staff and patient (OSHA).
• Records treatments administered in medical record.
• Records clinical documentation according to accepted regulatory, corporate and professional guidelines.
• Participates in weekly case conferences.
• Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
• Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
• Participates in in-service training programs.
• Responsible for assuring patient safety.
• Complies with state regulations and national standards
• All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
