About the position
The Clinical Liaison LPN at Lifespark Complete Senior Living serves as the primary communication and clinical operations contact for field-based Advanced Practice Providers (APPs) and Physicians. This role is essential in supporting clinical staff in delivering comprehensive senior primary care services, ensuring efficient operations while maximizing vocational satisfaction. The position involves a hybrid work environment, requiring travel to various senior living sites in the Twin Cities metro area.
Responsibilities
• Perform excellent customer service to clients, customers, partners, and colleagues daily.

• Document visits and coordination details in EMR.
,
• Direct communication with MD/NP/LM/Managers for urgent client needs.
,
• Assist with medication reconciliation, vital sign entry, and skilled interventions as ordered.
,
• Assist members with urgent needs within the scope of practice of LPN.
,
• Participate in team meetings and collaborate with the clinical team.
,
• Assist with exams/procedures as needed and provide coverage for other clinic staff when necessary.
,
• Respond timely to inquiries via telephone, EMR, Microsoft TEAMS, email, or fax.
,
• Provide team coverage to other departments as needed.
,
• Complete all documentation in a timely manner and ensure client record content is accurate.
,
• Maintain confidentiality to protect member's health information.
Requirements
• Licensed Practical Nurse diploma or certified medical assistant.
,
• Experience using technology including Microsoft Office, Smart Phone, Clinical Software platforms (EMR), and Internet Browsing.
,
• Previous hospital or clinic experience is highly recommended.
,
• Strong planning skills and creativity to solve problems.
,
• Ability to effectively communicate at all levels of the organization, both verbally and in writing.
,
• Ability to maintain composure and positivity in stressful situations.
,
• Organizational skills to prioritize multiple demands in a fast-paced environment.
