About the position

The part-time Home Care Scheduler at Mercy Home Health is responsible for scheduling clients' Home Health and Hospice services, ensuring clinician continuity and matching clinician competencies to client care needs. This role requires effective communication with patients, clinicians, and other stakeholders while exercising judgment in scheduling decisions. The position is remote, requiring 20 hours of work per week and one weekend per month.

Responsibilities
• Schedule clients' Home Health and Hospice services with clinician continuity.

• Identify client needs and match clinicians with appropriate competencies.

• Exercise judgment in scheduling and rescheduling patients effectively.

• Maintain optimal communication with patients, clinicians, clinical managers, physicians, and homecare coordinators.

• Provide coverage for multiple sites and/or Hospice support as needed.

Requirements
• Computer efficient with working knowledge of Microsoft Office Suite.

• Exceptional verbal and written communication skills.

• Experience in healthcare-related field or home care experience preferred.

• Strong attention to detail.
