About the position

The Licensed Social Worker (LCSW) position in Crisis & Triage Management at Kaiser Permanente in Honolulu, Hawaii, is a vital role that provides ambulatory behavioral health services. This includes evaluation, diagnosis, treatment planning, case management, and psychotherapeutic interventions for individuals, families, groups, and Intensive Outpatient Programs. The position is designed to support members in crisis and those seeking behavioral health services, ensuring they receive the appropriate level of care. The role is flexible, allowing for a hybrid work schedule that includes both in-office and remote work options. In this position, the social worker will assist with the administration, development, implementation, and evaluation of social work programs and policies. They will work as part of a team in the Call Center Advice Line, providing telephonic triage and assessment for members requesting behavioral health services. The social worker will also provide telephonic crisis management, coordinate admissions and discharges for psychiatric admissions, and conduct follow-ups for post-hospital and emergency room discharges. The role requires conducting clinical social work consultations, performing clinical diagnoses, risk assessments, and crisis interventions. The social worker will provide psychotherapy and counseling using a wide range of evidence-based interventions and modalities, including crisis intervention, brief therapy, cognitive and behavioral therapies, skills training, supportive therapy, stress management, and family therapy. They will interview members to evaluate their psychological and mental status, utilizing the latest DSM to diagnose patients and determine if they are in crisis or at risk. Additionally, the social worker will assess urgent or high-risk clinical situations, coordinate referrals to psychiatrists or other clinicians, and assist with admissions to hospitals or specialized facilities. They will also gather information, identify resources, and educate clients about behavioral health services. Participation in therapeutic and educational group programs, evaluation of treatment outcomes, and collaboration with interdisciplinary clinical team members are also key components of this role. Maintaining high clinical expertise and adhering to professional guidelines is essential, as is documenting clinical assessments and treatment plans in patient medical records according to departmental guidelines.

Responsibilities
• Assist with administration, development, implementation, and evaluation of social work programs and policies.

• Coordinate admissions and discharges for members requiring psychiatric admissions.

• Help patients enhance or restore their capacities for personal and social functioning.

• Participate in or assist with therapeutic and educational group programs, evaluating outcomes and effects of treatment.

• Research, collect, and record data related to behavioral and interpersonal problems of patients.

• Collaborate and consult with physicians and interdisciplinary clinical team members to support assessments and decision-making.

• Maintain high levels of clinical expertise and knowledge of current clinical conditions and interventions.

• Document clinical assessments, treatment plans, and interventions in patient medical records according to guidelines.

• Participate in clinical team meetings, departmental planning, and quality management activities.

• Assess patients in Emergency Departments on an after-hours basis as needed.

Requirements
• Masters Degree in social work from an accredited college or university.
,
• Passing score on the clinical level national board examination.
,
• Supervised clinical social work training as required by Hawaii Revised Statutes (HRS) 467E-7.
,
• Licensed Clinical Social Worker (Hawaii).
,
• National Provider Identifier required at hire.
,
• Basic Life Support certification required at hire from American Heart Association.
,
• Knowledge and experience in conducting assessments and diagnostic evaluations using the DSM.
,
• Demonstrated knowledge and skill in adaptability, change management, conflict resolution, decision making, group process facilitation, influence, interpersonal relations, oral communication, problem solving, quality management, teamwork, and written communication.
