About the position

At UW Health, we are seeking a dedicated Registered Nurse (RN) to join our team in the Diabetes Education Clinics. This part-time position offers a unique opportunity to support patients through their diabetes journey by providing care coordination, telephonic triage, and occasional in-person nurse visits. You will work closely with an interdisciplinary team that includes physicians, advanced practice providers, diabetes care and education specialists, medical assistants, and schedulers to ensure that each patient receives remarkable care. Your nursing knowledge and critical thinking skills will be essential in this role as you participate in process improvement and quality initiatives aimed at enhancing patient outcomes. This position is based at the 20 S. Park Clinic located in Madison, WI, and operates on an 80% FTE schedule during day shifts, with no weekends or holidays required. Some remote work options are available, providing flexibility to balance your professional and personal life. Responsibilities
• Support patients through their diabetes journey via care coordination and telephonic triage.

• Conduct occasional nurse visits for patient support.

• Collaborate with an interdisciplinary team including physicians and diabetes care specialists.

• Utilize nursing knowledge and critical thinking to ensure quality patient care.

• Participate in process improvement and quality initiatives.

Requirements
• Graduate of a school of nursing.
,
• Registration as a registered nurse in the state of employment or licensure in a state in the licensure compact.

• CPR/BLS Certification.
,
• Minimum of six months of RN experience.

Nice-to-haves
• Bachelor of Science in Nursing (BSN).

• Relevant RN experience.

• Applicable clinical certification.
