About the position

The Clinical Referral Specialist RN serves as a vital in-office clinical liaison for Sutter Health, specifically within the SCAH-Sutter Care at Home division. This role is primarily focused on managing patient referrals for home health services, ensuring that all interactions with referral sources are handled with professionalism and efficiency. The specialist will be responsible for receiving and processing referrals via telephone, which includes close collaboration with hospitals, physicians, skilled nursing facilities, and families. The goal is to facilitate timely admissions and ensure that patient care begins without delay. In this position, the Clinical Referral Specialist will enter referral information into the Electronic Medical Records (EMR) system and create initial orders to kickstart patient care. This role requires excellent customer service skills, as the specialist will frequently interact with various stakeholders, including other staff members and external referral sources. The position is remote but limited to applicants residing in Arizona, Idaho, or Utah, and it operates on a full-time schedule with day shifts from Monday to Friday, including every other weekend. Candidates must be graduates of an accredited nursing school and hold a valid RN license along with BLS certification. The role demands a minimum of two years of relevant experience, with a strong emphasis on clinical knowledge, critical thinking, and problem-solving abilities. Familiarity with ICD-10 coding is preferred but not mandatory. The specialist must demonstrate exceptional interpersonal skills and a commitment to teamwork and customer service, both internally and externally. Flexibility in scheduling is also required to accommodate increased workflow during peak times.

Responsibilities
• Act as the in-office clinical liaison for patient referrals.

Requirements
• Valid RN License in California.

• BLS Certification as a Basic Life Support Healthcare Provider.

• Minimum of 2 years of recent relevant experience in a clinical setting.

• Strong clinical knowledge and critical thinking skills for patient care coordination.

• Exceptional interpersonal and customer service skills.

• Ability to solve unique problems and consult supervisors when necessary.

• Knowledge of current Hospice/Home Health admission criteria and regulations.
