About the position
The Patient Care Coordinator role is a fully remote position focused on providing administrative support in a clinical setting. The coordinator will assist with patient discharges, manage communications from after-hours phone queues, and ensure smooth transitions for patients returning to in-network facilities. This position requires availability for after-hours work, including weekends and holidays, to meet the needs of patients and healthcare providers.
Responsibilities
• Assist with same day anticipated weekend discharges including appointments, home health, and durable medical equipment (DME).

• Repatriate members from non-contracted facilities to in-network facilities.

• Manage emergency department diversions to skilled nursing facilities (SNF) and home-like care (HLOC).
,
• Answer phones from the after-hours phone queue.

• Reply to emails regarding patient care and coordination.
Requirements
• High school diploma or GED required.

• 1 year of experience in healthcare required.

• 1 year of experience as a Patient Care Coordinator required.
