About the position
The Clinical Care Coordinator at Greater Oregon Behavioral Health, Inc. is responsible for providing high-quality, integrated, trauma-informed, and culturally competent care coordination services to members with complex behavioral health care needs. This remote position requires collaboration with various community-based providers to ensure that members' needs are met through the development of individualized care plans. The role also includes cross-coverage in Utilization Management and may require after-hours work.
Responsibilities
• Document and present coherent clinical assessments and case formulations.

• Develop individualized plans of care.
,
• Teach and provide telephonic support for members and their support systems.
,
• Coordinate with providers at all levels of care.
,
• Facilitate communication between members and their support systems.
,
• Participate in interdisciplinary or multidisciplinary team meetings associated with member care.
,
• Provide technical assistance and training to community partners regarding programs and systems of care.
,
• Perform cross coverage when required, fulfilling all duties of the Utilization Management Integrated Services Coordinator.
Requirements
• 5 years' experience in mental health.
,
• Knowledge of DSM criteria for mental health and substance dependence/abuse diagnoses.
,
• Experience with programs such as EASA, IIBHT, WRAP, or coordinating between levels of care with hospitals or DHS.
,
• Ability to conduct psychosocial assessments and formulate multiaxial DSM impressions.
,
• Strong communication skills to present, recommend, and listen effectively.
,
• Proficient in Microsoft Office software and/or Google Suites.
,
• Strong planning and organizational skills.
,
• Master's degree in social work, psychology, sociology, or a closely allied field.
,
• Licensed Clinical Social Worker or equivalent Oregon licensed healthcare profession.
