Bilingual RN Patient Care Coordinator

The ideal candidate will be confident and comfortable on the phone, bilingual in English and Spanish, and great at providing support both patients and caregivers.</p><p></p><p><strong>Key Responsibilities:</strong></p><ul> <li> Serve as the primary point of contact for caregivers, providing support, answering questions, and addressing any needs. </li> <li> Schedule patients and confirm upcoming appointments with accuracy and professionalism. </li> <li> Coordinate care services, ensuring seamless communication between patients, caregivers, and healthcare providers. </li> <li> Deliver excellent customer service and maintain a compassionate, patient-centered approach in all interactions. </li> <li> Maintain patient records in compliance with company policies and healthcare regulations.</li> </ul><p><strong>Requirements</strong></p><ul> <li>Current licensure as a Registered Nurse (RN) or medical degree.</li> <li> Bilingual English and Spanish. </li> <li> Strong communication skills, both verbal and written. </li> <li> Comfortable managing multiple tasks in a fast-paced environment. </li> <li>1 year of prior experience in patient care coordination, home health, or a similar healthcare setting. </li> <li> Proficiency with scheduling systems and basic computer applications.</li> </ul>
