Primary Care Physician – PACE Program

Location: Los Angeles
Schedule: Full-Time | Rotating On-Call (Remote)

What You'll Do

As a Primary Care Physician within the PACE model, you’ll provide comprehensive medical care tailored to the unique needs of older adults—helping them stay healthy, active, and in their homes as long as possible.

Key Responsibilities:
• Act as the primary care provider for an assigned panel of PACE participants
• Deliver direct medical care across multiple settings: PACE center, participants’ homes, and skilled nursing facilities
• Initiate, manage, and monitor medications, treatments, and labs
• Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans
• Provide ongoing support during hospital admissions, discharges, and transitions of care
• Educate participants and caregivers on chronic conditions, preventive care, and overall wellness
• Refer participants to specialty providers when needed
• Perform routine in-office procedures when clinically appropriate
• Maintain thorough, accurate documentation in the Electronic Medical Record (EMR)
• Participate in value-based care initiatives, quality improvement, and team development
• Contribute to broader organizational growth, innovation, and patient-centered outcomes

Schedule & On-Call
• Standard clinic hours Monday–Friday
• Rotating on-call responsibilities (evenings/weekends – remote only)

Qualifications
• Medical Degree (MD or DO) from an accredited institution
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Active and unrestricted state medical license
• DEA certification
• Spanish fluency or proficiency highly preferred
• Experience with older adult populations and/or interdisciplinary care teams is a plus
• Familiarity with value-based care models is desirable
• Strong communication, collaboration, and leadership skills
