The Community Health Worker position will be primary care manager for members with complex medical or behavioral needs who aren’t otherwise managed by the team’s clinicians.  The Community Health Worker works assertively to locate and engage members in the community.  They will develop individualized plans of care that focus on the members’ definition of success.  The Community Health Worker identifies and overcomes barriers to goal achievement, including SDOH barriers.  This position will work in collaboration with the interdisciplinary clinical care management team (Pod) to advance interventions aligned with a member’s readiness to change. The Community Health Worker will approach their member work with an understanding of how inequities drive health disparities. The Community Health Worker will promote health equity.<u1:p></u1:p><o:p></o:p>
Working Schedule: Schedule is Monday through Friday, any 8-hour between the hours of 8:00 am to 5:00 pm Eastern Standard Time.<u1:p></u1:p><o:p></o:p>
Location: Remote in the state of Florida.<u1:p></u1:p><o:p></o:p>

Primary Responsibilities: <u1:p></u1:p><o:p></o:p>
<ul>
<li style="font-family: Tahoma;">Partner with clinical care management team to manage complex members in the community<u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Locate and engage members in the community<u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Conduct screenings telephonically or virtually, to assess member medical, behavioral, functional, cognitive and psychosocial needs, i.e. health risk assessment<u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Engage members over the phone to educate about their health <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Review available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care) <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Conduct comprehensive member assessment that includes bio-psychosocial, functional, and behavioral health needs <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Utilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Identify when supplemental assessments are needed and conduct supplemental assessments <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Identify member service needs related to health concerns <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Identify urgent member situations and escalate to next level when necessary <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Engage members to participate in the assessment process and collaboratively develop a plan of care based on their individual needs, preferences, and objectives with nursing oversight <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goals <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Identify member support systems available and incorporate into plan of care <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Review plan benefits and identify appropriate programs and services based on heath needs and benefits <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Integrate health care and service needs into a plan or recommendation for member care and service <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Work collaboratively with the interdisciplinary care team to ensure an integrated team approach <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Collaborate with members to create solutions to overcome barriers to achieving healthcare goals <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Identify relevant community resources available based on member needs <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Refer members to appropriate programs and services <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Facilitate member choice of preferred provider <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Provide informal counseling and social support resources <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Advocate for individuals and communities within the health and social service systems <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Moderate work experience within own function <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Some work is completed without established procedures <u1:p></u1:p><o:p></o:p></li>
<li style="font-family: Tahoma;">Basic tasks are completed without review by others <u1:p></u1:p><o:p></o:p></li>
