About the position

Eventus WholeHealth is seeking a Medical Scheduler for our Primary Care/PSY float scheduling team. This position is integral to our mission of delivering holistic care to medically vulnerable adults, ensuring that their healthcare needs are met through effective scheduling and communication. As a Medical Scheduler, you will be responsible for coordinating facility and patient-level scheduling for all providers in assigned facilities or territories. This role requires you to ensure that care teams have scheduled routine visits, pain visits, medication visits, and any additional types of visits deemed medically necessary. You will play a crucial role in maintaining updated scheduling rosters and managing patient rosters to ensure timely adjustments for acute visits and day-of-visit schedule changes. In this position, you will communicate effectively with patients, facilities, medical assistants, care team coordinators, and providers to ensure that all patient needs are met. You will assist care teams in adjusting schedules as needed for PTO, sick days, or unusually high volumes of new patient referrals. Additionally, you will be responsible for reporting missed visits or provider concerns to the Care Team Clinical Manager, coordinating scheduling-related communication with the new patient registration department, and ensuring that all scheduled patient visits are communicated to the relevant facilities or patients. Your role will also involve documenting any appropriate CCM services and notifying Care Team Support Managers of any outstanding open notes by providers. The Medical Scheduler will work closely with Customer Account Managers to resolve scheduling-related facility issues and will be expected to check all forms of communication several times daily to ensure that patient needs are met. This position is essential in enhancing patient care and education through effective transitions of care communication with the CCM team. Responsibilities
• Coordinate facility and patient level scheduling for all providers in each of the assigned facilities/territory.

• Ensure Care Teams have scheduled routine visits, pain visits, and medication visits as needed.

• Ensure that each patient is seen as medically necessary, per clinical protocol.
,
• Update scheduling rosters daily to ensure accuracy.
,
• Manage patient rosters and ensure timely adjustments for acute visits and day-of-visit schedule changes.
,
• Assist Care Teams with adjustments for PTO, sick days, or high volumes of new patient referrals.
,
• Report reasons for missed visits or provider concerns to Care Team Clinical Manager.

• Coordinate scheduling/patient related communication with the new patient registration department and care teams.

• Assist with planning and evaluating patient registration methods with the patient registration team.
,
• Ensure facilities or patients receive copies of all scheduled patient visits and assist with problem-solving any issues.

• Document appropriate CCM services and notify Care Team Support Managers of any outstanding open notes by providers.

• Work with Customer Account Managers to resolve scheduling-related facility issues/concerns.

• Check all forms of communication several times daily to ensure patient needs are met.

• Assist with calls from facilities and patients regarding questions, issues, or concerns.

Requirements
• High School Diploma or GED required.
,
• 2 years of telephone or customer service experience required.
,
• 1 year of prior healthcare scheduling or medical-related experience required.
,
• Experience with an Electronic Medical Record (EMR) and/or practice management system.
,
• Familiarity with medical terminology and knowledge of HIPAA privacy laws required.
,
• Excellent PC skills required; strong Excel skills preferred.

• Competent knowledge of Microsoft Office; strong Excel skills required.

• Ability to work under pressure of tight deadlines and complete tasks accurately.

• Ability to effectively handle multiple priorities with detail and accuracy.
