About the position

The position involves coordinating case management for members in community long-term care facilities, ensuring high-quality care through assessment, evaluation, and the development of individualized care plans. The role includes providing consultation and education to healthcare providers, collaborating with special work teams, and offering direct primary, secondary, and tertiary care services in various settings, with a focus on long-term care.

Responsibilities
• Coordinates case management for members in long-term care facilities.

• Provides consultation and direct care to geriatric patients in inpatient, outpatient, and home care settings.
,
• Conducts intake assessments and develops individualized plans of care for members in long-term care facilities.
,
• Performs annual physical exams, timely certifications, and routine follow-up visits in collaboration with physicians.
,
• Documents all care provided, including observations, treatments, and communications according to standards.
,
• Orders and monitors laboratory and diagnostic tests.
,
• Educates patients and families about health status and treatment options.
,
• Collaborates with hospitals and clinics in program development and quality improvement initiatives.
,
• Identifies needs for inservice programs and participates in educational planning and evaluation.
,
• Acts as a liaison for facility and family concerns related to long-term care.
,
• Participates in patient care conferences with healthcare team members.
,
• Represents Kaiser Permanente at community events.
,
• Takes call for selected long-term care facilities on a rotation basis.

Requirements
• Minimum three years of direct patient care nursing experience, with one year in long-term care or geriatric specialty.

• Master's degree in nursing with a specialty in gerontological, adult, family, or acute care.
,
• Nurse Practitioner-Family Certification or equivalent certification required.
,
• Advanced Practice Registered Nurse License (Hawaii) and Registered Nurse License (Hawaii) required at hire.
,
• Basic Life Support certification from American Heart Association required at hire.

Nice-to-haves
• Valid Hawaii APRN Recognition with Prescriptive Authority.

• Experience working in Long Term Care (LTC) or Skilled Nursing Facility.

• Adult Gerontology, Palliative Care Certified NP.
