About the position

CalOptima Health is seeking a highly motivated and experienced Personal Care Coordinator to join our team. The Personal Care Coordinator (Bright Steps) will coordinate activities aligned with the Certified Perinatal Services Program (CPSP) and support CalOptima Health moms with trimester-specific, postpartum assessments, and infant assessments. The incumbent will ensure communication of the member's assessment and care plan with the member and the health care team. The incumbent will identify barriers to member's care and assist in improving these barriers for all levels of care. The incumbent will work closely with the health care team to ensure member access to timely services and coordination of care. In this role, you will participate in a mission-driven culture of high-quality performance, focusing on customer service, consistency, dignity, and accountability. You will assist the team in carrying out department responsibilities and collaborate with others to support short- and long-term goals/priorities for the department. Your responsibilities will include providing outreach to members to coordinate completion of trimester-specific assessments, conducting telephonic assessments, and collaborating with the health care team to ensure timely communication of member's clinical information. You will maintain documentation of member's case, care plan, and communication with the health care team within CalOptima Health's medical management system and ensure appropriate documentation of all interventions. Additionally, you will identify the need for referrals and coordinate with community resources or behavioral health services as needed. You will facilitate warm transfers in accordance with the member's needs when appropriate and notify the member's care team of key event triggers. You will guide members in understanding and accessing the benefits they are entitled to under Medi-Cal, maintain compliance with established departmental productivity guidelines, and provide productivity metrics reports to your supervisor as needed. You will also work with CalOptima Health staff from other departments to expedite the resolution of member concerns. This position is eligible for partial telework in California, allowing for flexibility in your work arrangement.

Requirements
• High School diploma or equivalent required.
,
• 1 year of experience working with maternal and child health programs required.
,
• Bilingual in English and in one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese) required.

Nice-to-haves
• Certification from California Board of Medical Assistant or American Registry of Medical Assistants.

• 2 years of experience working with maternal and infant health programs, including Comprehensive Perinatal Services Program (CPSP).
