You will collect and manage medical data related to members' chronic conditions and medications, inputting this information into our Electronic Medical Record (EMR) system. Responsibilities
• Become experts at our Hit the Ground Running (HTGR) Process to set our members up for success with their healthcare needs when they first enroll.

• Capture necessary chronic condition/medication and proactively enter them into the EMR.
,
• Input member data into appropriate outreach management lists in EMR to follow-up either daily, weekly, or monthly.
,
• Manage daily, weekly, and monthly outreach lists.
,
• Work with RDH Medical Providers on status of care/medical needs.
,
• Demonstrate strong attention to detail and an inquisitive drive to analyze information and data.

Requirements
• 1 Year of experience in a Healthcare Administrative Support role or Case Management-related role, preferred.
,
• 1 year of telephonic outbound with heavy call volume, preferred.
,
• Medical Assistant, Nurse Assistant or other medical experience preferred.
,
• Proficiency in using Microsoft Word, Excel, and an Electronic Medical Record system, preferred.
,
• Bilingual (Spanish/English), highly desired.
,
• Must be able to successfully pass National Agency Check with Inquiries (NACI) background investigation, required.

Nice-to-haves
• Experience in a medical specialty such as Dermatology, Ob/Gyn, Primary Care, or Urgent Care.
