Responsibilities
• Coordinate patient care plans in collaboration with the healthcare team

Requirements
• Proficiency in medical terminology and documentation

• Experience in behavioral health or related field

• Strong case management skills

• Knowledge of geriatrics and aging-related counseling

• Familiarity with medical records management systems

• Excellent interpersonal and communication skills
,
• Ability to provide compassionate support to patients and families

• 2 years of experience in social work (Preferred)
