As an Intake Coordinator LVN/LPN at AccentCare, you will play a crucial role in the healthcare journey of our patients and their families. In this hybrid role, you will work from the office on Fridays and Mondays, while Saturdays and Sundays will be remote. The working hours are from 8 AM to 7 PM, allowing for a structured yet flexible work environment. You will have the opportunity to collaborate with local branches in Longview, Carthage, Nacogdoches, Plainview, Lubbock, Midland, Odessa, Amarillo, Pampa, Clovis, and Woodward, TX, enhancing your professional network and experience. Responsibilities
• Coordinate patient intake processes and ensure accurate data entry.

• Communicate effectively with patients and their families to gather necessary information.

• Maintain knowledge of state and federal regulations and accreditation requirements.
,
• Provide support to the healthcare team in managing patient care needs.
,
• Assist in scheduling appointments and follow-up visits as needed.

Requirements
• Valid license as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN).

• Minimum of one year experience as an LVN or RN in a medical setting, preferably in home health.

• Strong knowledge of state and federal regulations and accreditation requirements.
,
• Excellent verbal and written communication skills, particularly over the telephone.
,
• Proficient data entry skills.

Nice-to-haves
• Experience in a corporate healthcare setting.

• Familiarity with electronic health records (EHR) systems.
